A Social Prescribing Link Worker is a non-medical health professional who works alongside our GP Surgery. They can provide you with a helping hand to achieve your social needs. They can help you access a wide range of support services provided by the NHS, County Council, Voluntary, Community and Social Enterprise (VCSE).
The aim of their role is to help you to improve your health, wellbeing and independence by linking you to groups and services in your local area. Together, they will identify what matters most to you and what you would like to work on together.
